FORMS On this page you will find the forms you need to submit your prescription order through the Canada Drug Company. These forms are in Portable Document Format (PDF) and require that you have the Adobe Acrobat Reader software installed on your computer to view and print them. If you do not have the Acrobat Reader software installed, you can obtain it for free from the Adobe website. We have provided a direct link to their website for you; just click on one of the "Get Acrobat Reader" buttons below and proceed with the installation. After completing it, you may then view and print the Canada Drug Company forms. If you need assistance in viewing, printing, or filling out the forms, please call us at (941)-650-5695. FIRST TIME ORDER If this is your (or your patient/client's) first time ordering through us, you will need the Complete Order Form package. This includes the Customer Account Profile, Limited Power of Attorney & Release, Medical History, and Order Form. There is also a review of the ordering process included. Click the "Complete Order Form Package" button to view and print it out. Once it has been filled out, you can send it us by fax or mail, or contact us to have it picked up. REFILL ORDER If you are an existing customer of the Canada Drug Company and want to order a refill of your prescriptions, call us at (941)-650-5695, or you can click on the "Refill Order Form" button to print it out. Then just complete it and send it to us. EXISTING CUSTOMER WITH NEW PRESCRIPTION ORDER If you are an existing customer of the Canada Drug Company and would like to order a new prescription, or if there are changes in your existing medication, please click on the "Refill Order Form" button to print it out and send it to us together with your new prescription(s).
