IS THIS A NEW ORDER?

If so, follow these instructions:

Step 1:   See your Doctor to obtain written prescriptions for your medications.  Make sure your prescription is current and valid.  Or you can have your doctor fax your existing prescription directly to us, saving time and another visit.

Step 2:   Get our order forms and our prices for your medications.

Where to get them?

• ask your doctor's office for them or
  

• ask your residential community or HOA administrator for them (they can obtain them from us) or
  

• call us directly at 941-650-5695 and we will provide them to you or
  

• download the forms from our website on THIS PAGE.
  

Step 3:   Complete the forms.

What needs to be filled out?

• the Health Profile and Physician Information (pages 1-2)
  

• the Order Form showing the medications you want to order and your method of payment (page 3)
  

• the Limited Power of Attorney & Release Form (page 4)
  

Step 4:   Send your completed forms together with your prescription to the Canada Drug Company.

How?

• ask your doctor's office or residential community/HOA administrator to submit them to us
  

• fax or mail them to us.  Our fax number and address are on THIS PAGE
  

That's it! Your order will be submitted to our affiliated Canadian pharmacy the same day we receive it from you.  You will be receiving it directly at your home 2-3 weeks later, delivered by the U.S. Mail.
